This is not a medical assessment.
It is a practical, supportive conversation focused on safety, changing care needs, daily routines, available support systems, and future planning considerations.
Every family situation is different. Some seniors may benefit from small adjustments and additional support at home, while others may need to begin exploring longer-term care options.
Our role is to help families better understand what may help now — while also planning thoughtfully for the future.
The In-Home Planning Visit includes:
